If your plan says no

• ◆Find out whether the denial is a formulary exclusion or a failed prior authorisation. Only the second is worth appealing.

On HSA and FSA specifically: eligibility generally turns on whether the expense is a qualified medical expense for your plan, and most providers here simply do not address it.
